1. Read packet of literature and listen to tapes

3. Schedule an appointment by calling:

4. Plan on about 1 1/2 hours for the first evaluation, and 1 hour for parent training of program activities.
5. Schedule revisit for four months from initial evaluation...
6. Keep in contact with CAN-DO through phone, fax, email and monthly reports.
7. Come to revisit appointment for re-evaluation and program
updates.

**Deposits required on all appointments.
Initial evaluation appointments require a deposit of $300.00. Re-evaluation and
other types of evaluations require a 50% deposit when the appointment is
scheduled. The balance is due at the time of the appointment (see consulting
fees above).
Cancellations must be made four weeks prior to appointment or deposits will be forfeited.
